2. Please print this form, fill it out and email it to us at info@katrinatique.com. We will review it and send you a return number. In the alternative, you may call us at (954) 752-4342 to obtain your return number.

3. In either case, you must return the form to us. This return form is not a shipping label. It is a form that must be inserted in your package to assure that you receive proper credit. Please write the return number (the one we provided you with) on the form before inserting the form in your package.
